保護者の方（未成年の方であっても）の方の代理予約もできかねます。他人のスマホを使って予約しないでください。

ジャスミンレディースクリニックでは、連携したLINEアカウントや電話番号に紐づけて個人情報や診療内容、検査結果等の管理を行っています。以前、スマホを貸した方が事件に巻き込まれるトラブルがありました。別人の診療情報や個人情報を他人が知ってしまうと誤診の危険や個人情報保護法に違反しますので、必ず自分のスマホを使って予約、受診をしてください。ご本人以外のスマホでの予約が発覚した場合は予約は無効になり、ご自身のスマホで予約を取り直していただきます。

• 連携できるLINEアカウントも一つのみです。

男性はクリニックに入室できません。付き添いでお越しいただいても、クリニックの外でお待ちいただきますのでご注意ください。

Caution

• This clinic can only provide support in Japanese.
• This clinic is for female only. So we’re afraid male are not allowed to enter.
• To prevent misdiagnosis,female patient themselves must be able to speak Japanese in order to see a doctor.
• Male interpreters are not allowed to enter this clinic because this clinic is for women only.
• If the patient cannot speak Japanese and cannot bring a female interpreter,please find other hospital which can accept English speaker by yourself.
